The consultation went well. The patient was happy. The clinician said, “Come back in three months.” Then the patient left without a booking, and the recommendation became a sentence in a note.
There is no open enquiry and no missed appointment. The diary simply never contains the future booking, so nothing looks wrong.
Separate recommendation from booking
A recommended follow-up is a clinical event. A booked follow-up is an operational event. Recording the first should create a visible expectation for the second without asking software to invent the interval.
Use automation as a backstop
Offer the next booking before the patient leaves when that fits the agreed clinical plan and the patient’s preference. Automation can surface the missing booking, prepare a reminder in the clinic’s voice, and stop when the patient replies or books.
Measure return, not messages
The sequence succeeds when the patient books, deliberately declines, or is handed to a person because the response needs judgement. Every other status is open work.
